On the Job Injury Procedures
Woonsocket Education Department strives to provide a safe and secure working environment for all employees. In the event a work related injury occurs while performing your employment responsibilities, Woonsocket Education Department will provide appropriate medical care and treatment to the injured employee.
EMPLOYEES
Life Threatening Emergencies
911 should be immediately called or employee should be taken to the closest emergency room.
Employees must complete the Employee On the Job Injury Report either online in the school nurses office or if after the school day has ended the pen and paper form available from Human Resources in McFee. The form must be completed within 24 hours of the occurrence. Human Resources can email the form in the event the employee is unable to return to work the next day.
Non-Life Threatening Emergencies
Injured employee must report their injury to both their building principal and school nurse by the end of their shift on same day of injury.
Employees must complete the Employee On the Job Injury Report either online in the school nurses office or if after the school day has ended the pen and paper form available from Human Resources in McFee. The form must be completed by the end of their shift.
Facilities staff who are injured after the school nurse has left for the day must report their injury to the Chief Operating Officer.
Provide any medical documentation of treatment and any work restrictions to Human Resources.
Forward all medical notes or bills that may have incorrectly been sent to you directly to Human Resources for proper processing.
If employee has lost any time from work, they must present a note from a medical provider releasing them to return to work with or without an accommodation directly to Human Resources.
If employee is released to work with accommodations, Human Resources will review job description and have a meeting with employee if needed to discuss.
SCHOOL NURSE
Provide the employee space to complete the Employee On the Job Injury Report on the Chromebook. If non-life threatening the employee should complete the form themselves before their shift ends for the day. If life threatening, the nurse should complete the pen and paper form on behalf of the employee to the best of their ability within 24 hours of occurrence.
If the employee needs more first aid you can provide, strongly encourage the employee to go immediately to Landmark Medical Center for urgent care at 115 Cass Avenue, Woonsocket, RI 02895.
Notify your building principal and Human Resources immediately of injury (401-767-4625).
Complete the nurse portion of the Nurse/Supervisor report and forward on to employee’s supervisor so they can complete their section and return to Human Resources.
BUILDING ADMINISTRATOR
If the school nurse is out, you may be the first person to learn of the injury, please notify Human Resources.
If employee is taken by ambulance please immediately notify the Superintendent’s Office, the Chief Operating Officer, and Human Resources.
Complete the supervisor’s portion of the Nurse/Supervisor report and forward on to Human Resources within 24 hours of the injury.
If employee misses time, enter absence into AESOP with a substitute as needed. Employees out on injury are required to use sick time unless they request in writing to use personal or vacation time.
HUMAN RESOURCES
Timestamp all injury reports submitted on paper.
Notify the Trust via online reporting system and then forward accident/incident forms to the Trust.
Record accident/incident data on internal/trending spreadsheet for analysis at the monthly Safety Committee meetings.
Notify Building Administrators of employee’s return to work date once established. Remind them to enter attendance into AESOP.
If employee will be out an extended period of time (two weeks or more) send a FMLA packet and letter of designation to them. Workers Compensation and FMLA must run concurrently and the employee must be notified.
Add employee to Medical Leave Spreadsheet for internal tracking.
When employee is ready to return to work forward letter clearing them for duty with or without accommodations to the Director of Human Resources & Labor Relations.
